Purpose:
To analyze refractive outcomes of wavefront-optimized (WFO) ablation and topography-guided custom ablation (TCAT) profiles using the EX500 excimer laser platform (Alcon Laboratories, Inc., Fort Worth, TX) in the treatment of myopia and myopic astigmatism.
Setting:
Private Clinic, Oporto, Portugal
Methods:
Twenty-eight eyes of 14 patients who underwent PRK were included. WFO ablation was performed in one eye (WFO group) and TCAT in the fellow eye (TCAT group). The WaveLight EX500 excimer laser (Alcon Laboratories, Inc.) was used for photoablation. The customized treatment plan was based on Topolyzer Vario topography system (Alcon Laboratories, Inc.) data. The patients were observed for 6 months after the procedure.
Results:
The surgery significantly reduced the manifest refractive spherical equivalent (MRSE), sphere, and cylinder (P < .05) at 6 months of follow-up in both groups. Accuracy, safety, and efficacy were similar in the two groups. The total root mean square (RMS) and RMS of lower order aberrations were significantly better in eyes that underwent TCAT (P < .05). The total root mean square (RMS) of the cornea was statistically higher (P = .04) in the WFO group compared to the TCAT group. No loss of lines of CDVA at 6 months postoperatively were reported in both groups.
Conclusions:
TCAT and WFO ablations provided similar outcomes after PRK for myopia and myopic astigmatism correction. Less induction of higher order aberrations were recorded following TCAT ablation.
