Abstract Details

Purpose:

Evaluate intraoperative performance and postoperative outcomes following cataract extraction with or without IOL implantation in children undergoing surgery for congenital cataracts upto 6 months of age

Setting:

Iladevi Cataract & IOL Research Centre, Ahmedabad, India

Methods:

Retrospective case series which included patients undergoing cataract surgery upto 6 months of age with a minimum followup of 18 months. Surgical strategy included manual anterior and posterior capsulorhexis alongwith bimanual irrigation/aspiration for all eyes. Intraoperative performance and postoperative complications, mainly incidence of glaucoma and significant visual axis obscuration(VAO) requiring secondary procedure were documented. Visual acuity was recorded for all children at final followup.

Results:

61 eyes(36 patients) were included. Only 11/61 eyes had unilateral cataracts. Mean age at surgery was 3.4 + 2.3 months. Mean followup duration from surgery was 34.7 + 9.53 months. 18% eyes were left aphakic and 82% were pseudophakic. All pseudophakic eyes had an IOL in the capsular bag or optic capture through posterior capsulorhexis. Until final followup glaucoma developed in 8.1% eyes, of which 3 needed glaucoma surgery. All eyes that developed glaucoma were <4 months at time of surgery. Visual axis obscuration(VAO) was seen until last followup in 4/61 eyes(6.55%).

Conclusions:

Cataract surgery under 6 months of age is safe with acceptable rate of complications. IOL implantation in the capsular bag appears to be safe in this age group.
