Purpose:
This study aimed to objectively test and compare the optical quality of three trifocal intraocular lenses (IOLs) in polychromatic light with various degrees of decentration and tilt as well as the effect of the pupil size.

Methods:
An optical bench device (OptiSpheric® IOL Pro2) was used to study the following trifocal IOL models: AcrySof IQ PanOptix (Alcon), AT LISA tri 839MP (Zeiss) and FineVision POD F (PhysIOL). Five IOLs per model with power of 22.0D were analyzed. A polychromatic light filter was used to simulate the spectral sensitivity of the human eye. The optical performance was assessed by means of the modulation transfer function and the area under the MTF. The image quality was measured with an aperture size ranging from 2mm to 5mm and defocus ranging from 0 to 4D. Decentration and tilt were also tested.
Results:
All studied IOLs proved multifocal spanning an extensive range of distances. At 3mm, the MTF area of the Pod F and the AT LISA was comparable and larger than that of the PanOptix at far. At intermediate, the PanOptix demonstrated an extended focus area and a better optical performance than the other models. The Pod F yielded the largest MTF area at near. All IOLs showed high tolerance to tilt. Unlike the AT LISA tri, only limited effects of decentration were found in the Pod F and the PanOptix. Through focus MTF showed clear pupil effect on the IOL performance.
Conclusions:
The Pod F and the AT LISA demonstrated a comparable optical quality in the polychromatic light. Although the PanOptix showed an enhanced intermediate range, it was at the expense of other distances. The IOLs differed in their tolerance to decentration, but tilt had no important effects. The pupil size significantly affects the image quality.
