Abstract Details
Purpose:
To study the in vitro activity of antibiotics against microorganisms in the conjunctival flora of patients posted for cataract surgery. This was done to find out the ideal perioperative antibiotic that should be used in patients undergoing cataract surgery to prevent endophthalmitis.
Setting:
The study was done in a tertiary eye hospital in South India.
Methods:
Conjunctival swab was taken from Six Hundred and Seventy Six (676) patients who were posted for cataract surgery between November 2016 and February 2019. Culture and sensitivity was done using these samples using standard microbiological techniques. Antibiotic susceptibility was studied in the samples which grew bacteria.
Results:
301 samples grew Coagulase Negative Staphylococcus and the highest degree of resistance was seen to Azithromycin (49.5%) and the least to Chloramphenicol (10.6%) and Tobramycin (6.6%). (29.9%) were resistant to Moxifloxacin. 57 samples grew Staphylococcus aureus of which the highest resistance was seen to Azithromycin (56.1%) and least to Chloramphenicol (8.7%) and Tobramycin (1.7%).(42.1%) were resistant to Moxifloxacin.17 samples grew Klebsiella pneumoniae and were most resistant to Moxifloxacin 41.1% (7/17) and least resistant to Amikacin, Azithromycin and Chloramphenicol (11.7%). Two samples grew Pseudomonas Aeruginosa and was resistant (1/2) to Moxifloxacin and Chloramphenicol alone.
Conclusions:
Organisms in the conjunctiva flora are most sensitive to Tobramycin and Chloramphenicol. The highest degree of antibiotic resistance was seen to Azithromycin. There is a high incidence of resistance to moxifloxacin also in organisms grown from conjunctival sac. Moxifloxacin is thus not a good choice for perioperative antibiotic prophylaxis for cataract surgery. Tobramycin and Chloramphenicol are the most effective antibiotics for perioperative antibiotic prophylaxis in patients undergoing cataract surgery to prevent postoperative endophthalmitis.
