Incidence of corneal ectasia in patients undergoing ReLEx SMILE and ReLEx SMILE Xtra in a high-volume tertiary refractive surgery centre

Purpose:
To report the incidence of corneal ectasia in eyes with normal and borderline topography undergoing ReLEx SMILE and ReLEx SMILE Xtra (SMILE combined with simultaneous crosslinking) in high volume tertiary refractive surgery center from India.
Setting:
Nethradhama Super Speciality Hospital, Bangalore.
Methods:
In this retrospective study, corneal topography of all patients who underwent ReLEx SMILE or ReLEx SMILE Xtra from January 2015 to June 2018 were analysed. Of the 4630 eyes analysed, 698 eyes were categorised into borderline and 3932 eyes into normal topography. Amongst the 698 borderline eyes, 334 eyes underwent ReLEx SMILE Xtra and 364 eyes underwent only ReLEx SMILE. The corneal topographies were categorised into normal and borderline topography based on criteria such as age, corneal thickness, residual bed thickness, I/S asymmetry and Belin Ambrasio display.
Results:
Incidence of corneal ectasia in the borderline group was 1.16%, as 5/698 eyes in this group reported ectasia at a mean duration of 21.3 months (1.7 yrs). All the 5 ectasia eyes had undergone ReLEx SMILE without crosslinking, however borderline eyes which had undergone ReLEx SMILE Xtra (334 eyes) remained stable and did not show any evidence of ectasia (0%). Also 3932 normal topography eyes which underwent ReLEx SMILE showed no ectasia (0%). Overall incidence of ectasia in eyes undergoing ReLEx SMILE was 0.17% in 3 yrs duration study.
Conclusions:
Results of this study indicate that SMILE combined with simultaneous crosslinking (ReLEx SMILE Xtra) may be a valid option in treating borderline corneas to prevent future ectasia
