Abstract Details
Purpose:
To compare the early visual and optical quality recovery according to the postoperative residual stromal bed (RSB) and the lenticule thickness (LT) after Small-Incision Lenticule Extraction (SMILE) at low energy level for moderate and high myopia and myopic astigmatism treatment.

Lyon Vision Clinic, France
Methods:
This prospective study included 69 eyes of 69 patients with a total residual uncut stroma more than 360µm and separated into two groups: RSB from 200 to 250µm (Group 1: 20 eyes) and RSB more than 250µm (Group 2: 49 eyes). Each group was divided in two sub-groups according to the LT: 100 to 150 µm (Group 1A and Group 2A) and 150 to 200 µm (Group 1B and Group 2B). Visual acuity (VA), Contrast Sensitivity (CS), Corneal High-order Aberrations (HOAs), Modulation Transfer Function Cut off (MTF cut off), Objective Scatter Index (OSI) were measured.
Results:
All eyes recovered 20/20 or better of CDVA and the preoperative CS values at 1M in group 1 and 2. At 1 month, in Group 1 and 2, MTF cut off was slightly lower than the preoperative value (respectively 39.99 to 35.15 and 39.17 to 32.69) and OSI was slightly higher than the preoperative value (respectively, 0.76 to 1.11 and 0.75 to 1.05). No differences in the optical quality measures (Corneal HOAs, MTF, SR and OSI) were found between each group of RSB and between each subgroup of LT in preoperative and at 1D, 8D and 1M after SMILE.
Conclusions:
The classic RSB of 250µm in LASIK is debating in SMILE. This study showed no difference of early visual and optical quality recovery according to the RSB during the first postoperatively month. Further long term studies would be needed to confirm these results and the safety of RSB less than 250µm.
