ReLEx SMILE treatment of post cataract surgery anisometropia of 73 year old man: 30 months follow up

Abstract Details

Purpose:

To evaluate safety, stability and predictability of Relex Smile surgery on senile cornea as an alternative for treatment of post cataract surgery anisometropia.

Setting:

Oftum laser eye surgery clinic, Astana, Kazakhstan. Tomas Juhas jr. MD, PhD., Tomas Juhas sr. MD, PhD., Aizhan Aliyakparova MD.

Methods:

73 years old man 4 months after bilateral cataract surgery with significant anisometropia on dominant right eye refusing further intraocular surgery came to our clinic for laser treatment. Considering age, cornea, Schirmer test, visual acuity and refraction we performed -2,5sph -2,5cyl ax 95 low energy Relex Smile with standard settings - 120um thick 7,5mm large cap, 6,5mm wide lenticule, 0,1mm transition zone and 2,5mm wide incision at 115 degrees. Video was recorded. Refraction, UCVA, BCVA, Corneal healing, Schirmer test, Pentacam analysis, fundus finding were evaluated pre- and postoperatively at day 1, day 7, one month and 30 months after ReLex.

Results:

Refraction changed from initial -2,5sph -1,75cyl x 96 to 0,00sph -0,75cyl ax 54, -0,5sph -0,25cyl ax 43, +0,25sph -0,5cyl ax 85 and -0,25sph -0,00cyl 30 months after surgery. UDCVA changed from 0,1 to 0,4(-2), 0,7(-1), 0,6 and 0,8. BCDVA changed from 0,7 with -2,5sph -2,5cyl x 95 to 1,0(-1) with -0,25sphere. Healing was normal. Schirmer test 5mm at all visits. Pentacam stable,  central pachymetry changed by 70um from 532 to 462 um, 15 um less than calculated maximal lenticule thickness, patient uses reading glasses +2,5sph bilateral and is happy. Retina still very good, no AMD or glaucoma changes.

Conclusions:

We achieved desired result with accurate and stable refraction and UCVA even better than preoperative BCVA. Healing of the cornea and stabilisation of a result was similar to standard Relex Smile cases. Relex Smile is painless, gives fast result, as a mini invasive flap-less technique avoids flap related and epithelium related complications. Correction of post cataract surgery anisometropia with Relex Smile is now our first choice refractive laser solution when no corneal contraindications are present.
