Causes and outcomes of piggyback IOL for management of refractive errors after cataract surgery

Purpose:
To consider the etiology of surprise IOL powers after non complicated Cataract surgery and evaluate the outcomes of secondary piggyback IOL implantation in these unsatisfied patients
Setting:
Ophthalmic Research Center,Labbafinejad Medical Center, Shaid Beheshti University of Medical Science,Tehran, Iran
Methods:
This prospective case series included 9 pseudophakic eyes of 9 patients, who had residual myopia or hyperopia after cataract surgery. Data was collected preoperatively and post operatively and included age, sex, BCVA and UCVA after first cataract surgery, post cataract surgery refraction , the type and power of the secondary piggyback IOL implantation and UCVA after secondary IOL.
Results:
patient’s age ,range from 6m to 71y.Causes for secondary piggy back IOL implantation were myopic surprises in two eyes and hyperopic in 7 eyes
Cause for IOL surprise in seven eyes were miscalculated IOL power in 7 eyes.In 2 others ,myopic shift after childhood cataract surgery . Post-operative target refraction were Plano except for three children that power were adjusted for age and management of aniometropia.Six months after piggyback IOL implantation 78 % of eyes were within +/- 0.5D from target refraction .
No intraoperative or post operative complications were observed during flow up period..
Conclusions:
Bases on current study secondary piggy back IOL is an effective and safe technique to correct residual ametropia, specially in extreme hyperopia and children with significant anisometropia.
