Traumatic aniridia and aphakia management using combined vitrectomy and ab-externo scleral fixation of an artificial iris intraocular lens (IOL) using Gore-Tex suture

Abstract Details

Purpose:

To present a new surgical technique of treatment in a patient with severe iris trauma and aphakia combining vitrectomy and Gore-Tex (W.L. Gore & Associates, Newark, DE) suture 4-point scleral fixation of an artificial iris IOL.

Setting:

The case refers to a 55 years-old male having history of severe ocular trauma with subtotal iris absence and crystalline lens loss and been treated initially with vitrectomy and silicone oil tamponade. The silicone oil had undergone emulsification and had been partially removed. Silicone oil glaucoma was present.

Methods:

A variety of measurements were taken using the IOL Master (Carl Zeiss Meditec, Jena Germany) and photos from the fellow eye. Using a hybrid 25-27 gauge vitrectomy setting, the silicone oil droplets were removed. Then with the help of a Gore-Tex suture, knots were made ab externo to the haptics of an artificial iris IOL (Ophthec 311, Groningen, the Netherlands), the lens was fixated to the sclera and the sutures were pushed inside the eye.

Results:

No suture-related or other serious complications were noticed. The visual acuity improved to 20/30. The intraocular pressure (IOP) was controlled using Dorzolamide-Timolol eye drops. The functional and cosmetic results were very satisfactory.

Conclusions:

Combining modern vitrectomy environment with artificial iris IOL and Gore-Tex suture scleral fixation in eyes with partial or total absence of the iris and the lens, seems to provide a holistic and long-term anatomic and functional restoration.
