Early effectiveness of topical cyclosporine 0.05% after cataract surgery according to the severity of dry eye

Abstract Details
Purpose:
To evaluate the effectiveness of topically administered 0.05% cyclosporine in early postoperative period after cataract surgery, and to compare the therapeutic efficacy according to the severity of dry eye.
Setting:
Pusan National University College of Medicine, Medical Research Institute of Pusan National University Hospital, Pusan, Republic of Korea
Methods:
One hundred fifty-six patients who underwent unilateral cat¬aract surgery and received topical cyclosporine for 8-weeks were classified into three groups according to preoperative dry eye level; non-dry eye as control group; level I dry eye as group 1; and level II dry eye as group 2. The dry eye symptom questionnaire (ocular surface disease index, OSDI), tear film break-up time (TBUT),intraocular pressure(IOP), visual acuity, and Schirmer test-I (STI) were examined.
Results:
The preoperative dry eye symptom score improved significantly at postoperative 1-week and continuously improved until postoperative 8-weeks in all three groups, especially Group 2 compared with control group. Group 1 and 2 showed significant improvement in the TBUT at 1-week, 4-weeks, and 8-weeks postoperatively. At 4- and 8-weeks postoperatively, significant difference in the magnitude of TBUT improvement was observed only between the Group 2 and the control group. Postoperative STI changes showed no significant differences among the three group at all postoperative periods.
Conclusions:
Topical cyclosporine 0.05% after cataract surgery is more effective in dry eyes compared to non-dry eyes in improving BUT and dry eye symptoms. The more the preoperative dry eye is severe, the more the therapeutic effect is obtained during the early postoperative 8-weeks.
