Cataract management in an eye with residual astigmatism after multiple corneal surgeries

Purpose:

Case analysis: To show how we can best manage a residual astigmatism after multiple corneal surgeries.

Setting:

University Clinic Frankfurt, Goethe University, Theodor-Stern-Kai 7 | 60590 Frankfurt am Main, Germany

Methods:

A 61-year-old with history of keratoconus and penetrating keratoplasty surgery in both eyes. He proceeded to LASIK in his OS because of refractive. In 2010, he had OS refractive error of -4.0sph/-1,50 cyl/120°, so he had a FALAK for better optical quality. In 2017, a OD refractive error of 0,00sph/-3,oo cyl/81° and OS refractive error of -4,00/-2,5 cyl/118° was diagnosed. Slit lamp showed lens opacity. Pachymetry showed irregular astigmatism and corneal thickness of 529µm OD and 443µm OS. BCVA OD was 0,50logMAR and BCVA OS was 0.40logMAR. Patient requested emmetropy. We suggested a FALCS with implantation of monofocal-spherical IOL.

Results:

During surgery no adverse events occurred and one week postoperatively the UCVA OD was 0.40logMAR and UCVA OS was 0.50logMAR, a refractive error of +1,75 sph/-0,50cyl/158° in OS occurred and the BCVA was 0.20logMAR.

Conclusions:

It is widely known that it is difficult to calcuate the correct lens power in patients who have had multiple corneal surgeries. It is even more difficult to achieve acurate lens power calculations and determine the correct lens for these patients when a multifocal IOL is desired. For patients with irregular refraction or corneal finding, our study suggests that a monofocal lens implant is best for patients with a history of corneal surgeries to achieve the desired emmetropy.
