Gonioscopically-assisted transluminal trabeculotomy (GATT): The Birmingham Experience

Purpose:
Describe the experience and results with using the GATT technique to effectively treat glaucoma
Setting:
Birmingham Midlands Eye Centre
Methods:
49 eyes with POAG, Juvenile Glaucoma and secondary glaucomas due to uveitis and steroid use were included. IOP was measured preoperatively as well as postoperatively on day 1, month 1,3,6 and 9. Mean medication usage and IOP reduction were calculated.
Results:
Mean preoperative IOP was 28mmHg (SD+/- 11mmHg) with a post GATT IOP of 17mmHg on day 1, 17.7mmHg at month 1, 16.7mmHg at month 3 ,16.3mmHg at month 6 and 14.2 at month 9. Percentage IOP reduction at 9 months was 34 %. Mean number of topical medication preoperatively was 3 (SD+/- 0.8) with a mean number of drops reduced to 2 at 9 months follow up (N= 8 patients). 17 patients were on acetazolamide preoperatively with this figure reducing to only 2 patients at month 9. Fifteen patients had hyphaema; which resolved by week 4.
Conclusions:
GATT appears to be highly effective at lowering IOP certainly in the short to medium term. Patient with secondary glaucomas; uveitic, steroid induced, and Juvenile glaucoma do particularly well.
