Abstract Details
Purpose:
To describe a case of orbital and conjunctival nontuberculous mycobacteria (NTM) infection presenting as refractory red eye.
Setting:
Taipei City Hospital, Taipei, Taiwan
Methods:
Case report.
Results:
A 64-year-old female developed refractory red eye two weeks after trauma by her comb. Salmon-patch lesion of bulbar conjunctiva was noted. Biopsy and histological exam revealed granulomatous inflammation with microgranuloma, and acid-fast bacilli were found, identified as Mycobacterium abscessus five weeks later by culture. Orbital CT showed enhancement along inferior sclera after contrast medium injection. Due to persisted bulbar conjunctival thickening and abscess-like lesion formation, excision of conjunctival and orbital granuloma with amniotic membrane transplantation twice were performed. Conjunctiva inflammation subsided after 4 month topical and oral antimycobaterial treatment. The presentation, diagnosis and treatment of ocular NTM will be discussed.
Conclusions:
NTM can cause infections of all adnexal and ocular tissues in patients with ocular trauma or surgical history. The pathologic findings were granulomatous inflammation without true caseation. Periocular cutaneous, adnexal and orbital NTM infections remain rare and require surgical debridement and long-term parental antibiotic therapy.
