Purpose:
To present a case of Radial keratoneuritis with wessely ring
Setting:
Cornea and external eye disease service, Department of Ophthalmology, University Hospital of Heraklion, Crete, Greece
Methods:
Case report
Results:
A 39 years old male, with negative previous history of ocular pathology or contact lens use, referred to our department with severe ocular pain and redness on his left eye. The patient reported unresponsed to medication prescribed elsewhere. Slit lamp biomicroscopy revealed the presence of a round corneal epithelial defect along with ring infiltrate and radial keratoneuritis. After 2 days of wash out with preservative free artificial tears, corneal culture was negative. Confocal microscopy did not reveal evidence of fungal or acanthamoebal keratitis. he patient responded to antifungal treatment as well as second stage therapy for promotion of epithelial healing
Conclusions:
We present a case of a presumed fungal keratitis presenting with radial keratoneuritis and wessely ring with both negative cultures and confocal microscopy findings. Even though radial keratoneuritis is highly suspicious of Acathamoeba keratitis, when not proved, treatment should be initiated according to patient specific signs, symptoms and healing response.
