Safe and effective performance of collagen cross-linking for the treatment of progressive keratoconus by specialist nurse: review of one-year practice results

Abstract Details
Purpose:
The aim of this review is to demonstrate safety and effectiveness of Collagen Cross Linking (CXL) performed by a suitably trained Specialist Nurse (SN) within a Consultant led Service for the Treatment of Progressive Keratoconus.
Setting:
Retrospective review of all cases of CXL, performed by our SN over a period of one year.
Methods:
Data was collected from the dedicated Operation Theatre record and complemented by tests and measurements during subsequent Outpatient appointments. Eleven of the performed cases were tertiary referrals and therefore followed up in other departments.
Results:
Our SN was offered training on accelerated Protocol by the Service Leading Consultant and performed 20 cases partly/ assisting & 20 cases fully under supervision prior to treating cases independently.
34 cases were performed in the first year with male/female ratio 19/15. There were no post-operative complications.
61% of cases showed improving or stable corneal topography consistently post-operatively. 26% of cases showed deterioration of one corneal meridian by 1D or more 2-5 months post-operatively on Pentacam with one case deteriorating more than 2D at 3-month follow up. 13% of cases showed deterioration of 1.2-1.4D between 6 and 12 months post-operatively.
Conclusions:
The study shows safe and effective performance of CXL for the Treatment of Progressive Keratoconus by our SN with growing number of cases and adequate follow-up time. Such practice ensures reduction of patients’ waiting time, smoother running of the Anterior Segment Service and higher level of engagement provided to our Nursing Staff, which in turn can boost motivation and improve performance.
