Abstract Details
Purpose:
To evaluate the clinical outcomes of 134 consecutive cases that underwent DMEK surgery with surgeon prepared grafts in a tertiary center without a corneal bank.

Methods:
The medical records of the first 134 consecutive DMEK cases performed between 2010 and 2013 in a tertiary referral center were retrospectively reviewed. Indication for surgery, vision-limiting ocular comorbidities, pre- and postoperative best-corrected visual acuity (BCVA), manifest refraction, central corneal thickness (CCT) and endothelial cell density (ECD), as well as intra- and postoperative complications were evaluated before and 1,3,6,12 and 24 months after surgery.
Results:
CDVA improved from 0.71 ± 0.42 logMAR before DMEK to 0.19 ± 0.25 logMAR 3 months after surgery (P < .001). ECD decrease rate declined after the initial postsurgical decrease(38% at 3 months, 49% at 2 years), from 2430 ± 208 cells/mm² before DMEK to 1240 ± 440cells/mm² at 2 years. CCT decreased from 712 ±
120µm before DMEK to 547 ± 65µm at 2 years, with
a minimum (518 ± 41µm) at 3 months.
Conclusions:
DMEK assures a fast visual rehabilitation and maintains its clinical outcome within
a follow-up of 2 years. Visual acuity, central corneal thickness and endothelial cell density remain stable between 3 months and 2 years after surgery.
