Purpose:
To report the usefulness of superficial keratecomy, cynoacrylate glue and a bandage contact lens in the treatment of fungal keratitis not responding to medical treatment.
Setting:
Al-Hussaini Hospital, Assiut, Egypt
Methods:
A 53-year- old Egyptian woman presented with a recurrent herpetic stromal keratitis with an extensive epithelial defect in her right eye. She was on topical Aculovir ointment, fluconazole drops and moxifloxacin eye drops. Direct smears stained by Gram and Giemsa showed fungal elements. Culture was negative for both bacteria and fungi. I started treatment systemic acyclovir and topical Natamycin and gatefloxacin eye drops. Repeated scraping was done with no improvement. After 3 months superficial keratectomy was done with gluing the corneal perforation.
Results:
Superficial keratectomy plus cyanacrylate corneal glue were effective in controlling a resistant fungal infection not responding to medical treatment. A dense vascular corneal was left. The patient is scheduled for keratoplasty.
Conclusions:
Superficial keratectomy combined with cyanoacrylate gule can be considered a viable option for extensive fungal keratitis not responding to medical treatment.
