Comparison of toric and non-toric intraocular lenses in cataract patients with small magnitude of astigmatism in both eyes

Abstract Details

Purpose:

To study whether implantation of a toric IOL for the correction of small magnitude of astigmatism during cataract surgery results in better outcomes than a implantation of a non-toric IOL. .

Setting:

THESSALONIKI ORASI- Ophthalmic Microsurgery Day Clinic

Methods:

We studied both eyes of 18 consecutive patients with small amount of with the rule, oblique or against the rule astigmatism who underwent cataract operation by the same surgeon using a toric IOL (Alcon Acrysof SN6AT2 Toric) in one eye and non toric (Alcon Acrysof SN60WF) IOL in the other eye. Two eyes in the Toric IOL group and two eyes in the non-Toric IOL group had pre OP astigmatism of 1-1.37 Dpt and 16 eyes less than 1.00 Dpt. Differences in visual acuity, refraction, keratometric and topographc readings pre- and post-operatively between the 2 eyes.

Results:

Pre-operatively, mean corneal astigmatism was higher (0,68Dpt, sd=0,4Dpt) in the eyes implanted with a toric IOL and lower (0,48Dpt, sd=0,3Dpt) in the eyes corrected with a non toric IOL (p value=0,0026). Mean Snellen uncorrected visual acuity postoperatively was better (9,1/10) in the eyes with toric IOLs than with non toric IOLs (8.6/10) (p value=0,1). The mean refractive astigmatism postoperatively was significantly lower (p value=0,0020) in the eyes implanted with a toric IOL (0,31 Dpt, sd=0,17Dpt) than in the eyes with non toric IOLs (0,64Dpt, sd=0,34Dpt). Patients mentioned better quality of vision at the eye with the toric IOL.

Conclusions:

In patients undergoing cataract surgery, post-operative astigmatism was lower and quality of vision was better in the eyes implanted with a Toric IOL than the eyes with the non-Toric IOL. Taking into account that pre-operative astigmatism was statistically significantly higher in the Toric IOL group we can conclude that a implantation of a Toric IOL gives better results than a non-Toric IOL even in eyes with low levels of astigmatism (less than 1.25 Dpt).
