Torsional phacoemulsification and intraocular lens implantation without ocular viscoelastic device

Abstract Details
Purpose:
To compare clinical results of torsional phacoemulsification and intraocular lens (IOL) implantation with and without ocular viscoelastic device (OVD).
Setting:
Department of Ophthalmology, School of Medicine, Namik Kemal University, Tekirdag, Turkey
Methods:
In this randomized controlled clinical trial, eyes with moderate-density nuclear cataract were assigned to have torsional phacoemulsification and intraocular lens implantation with or without OVD. The main outcomes included corrected distance visual acuity (CDVA), central corneal thickness (CCT), central endothelial cell density (ECD), total ultrasound time (UST), cumulative dissipated energy (CDE), and balanced salt solution (BSS) volume. Postoperative follow-up was at 1 day, 1 week, and 1 and 3 months.
Results:
Each group (OVD and BSS) comprised 32 patients (32 eyes). The mean CDVA was similar in both groups during the postoperative follow up (P.334). In postoperative day 1, CCT was significantly high in BSS group (P=.001) while, it returned to the preoperative level in postoperative month 3 (P=.695). During the postoperative follow-up, the percentage of mean endothelial cell loss in both groups was between 17.3% and 21.9%; there was no statistically significant difference between the groups (P;.05). The mean CDE, UST, and BSS volume values were similar in the 2 groups (P;.05).
Conclusions:
Although CCT has significantly increased after torsional phacoemulsification and IOL implantation without OVD in the early period, the impact of the surgery on CDVA and endothelial cell loss is similar in both groups.
