Utility of crystal toric mobile app in the marking of corneal axis

Purpose:
To compare the degree of correspondence discrepancy in corneal axis marking between classical slit lamp manual marking and crystal toric application
Setting:

Methods:
Prospective study in 100 eyes (50 right and 50 left) of 50 patients attended at the office. We marked at slit lamp the horizontal axis (180) in half of each group and an oblique axis (135 for right eyes and 45 for left eyes) in the other half and we compared those axes with those suggested by crystal toric app
We calculated the degree of discrepancy between the two measuring systems and also whether the possible differences between the horizontal and the oblique group measurements were statistically significant
Results:
Mean angular deviation between both measurement systems was 2.34 degrees ± 0.69 (1.12 ± 0.38 for horizontal axis and 3.56 ± 0.77 for the oblique)
proportion of individuals with a discrepancy greater or equal to 3 degrees in the horizontal measurement was 22% (± 12%) while 58% (± 14%) in the oblique one. this proportion descended to 2% (± 3%) and 30% (± 13%) respectively if we use a threshold greater or equal to 5 degrees.
In both cases horizontal oblique differences were statistically significant (chi squared test p value 0.05)
Conclusions:
Crystal toric application may be a good tool for corneal axis marking previous to toric lens implantation.
Differences observed between the horizontal and the oblique groups measurements lead us to think that it could be due to mistakes in manual marking as it is more difficult to perform this hands up marking in the oblique axis.
Comparative studies with nowadays gold standard techniques (callisto verion) would be advisable in order to confirm data observed in this work
