Reactivation of keratoconus following cataract extraction surgery

Abstract Details
Purpose:
To present an interesting case of a patient with reactivation of keratoconus after an uncomplicated phacoemulsification surgery.
Setting:
Department of Ophthalmology, University Hospital of Alexandroupolis, Greece
Methods:
A 38-year-old male with bilateral keratoconus and pigmentary retinopathy underwent uncomplicated phacoemulsification in his recessive eye due to posterior cortical cataract. Corneal ectasia was observed following cataract surgery and was attributed to reactivation of keratoconus. Pre- and postoperative clinical findings and topographies are presented along with a literature review regarding the reactivation of keratoconus.
Results:
Preoperative UVA was <1/20 in his right eye while cataract was classified at stage 3 according to the Lens Opacities Classification System III (LOCS-3) grading scale. An uncomplicated cataract extraction surgery was performed. Preoperative K1 was 40,5D, K2 was 41,8D, astigmatism was 1,3D and thinnest corneal thickness (TCT) was 503μm, while five months postoperatively, K1 was 43,0D, K2 was 48,2D, astigmatism was 5,2D and TCT was 337μm.
Conclusions:
Postoperative keratectasia could be attributed to reactivation of keratoconus and should be taken into consideration prior to cataract surgery.
