Efficacy of cataract surgery combined with concurrent trabecular micro-bypass stent implantation for patients with open-angle glaucoma

Purpose:
The aim of this study was to evaluate the efficacy of a cataract surgery combined with concurrent trabecular micro-bypass stent implantation compared to a cataract surgery alone for reducing intraocular pressure (IOP), glaucoma medication and facilitating improvement in visual acuity to patients with open-angle glaucoma (OAG).
Setting:
Department of Ophthalmology, University Medical Center, Johannes Gutenberg University Mainz.
Methods:
This was a retrospective case series study including 25 eyes with concurrent open-angle glaucoma and cataract in each study group and control group. All patients underwent a cataract surgery alone (control group) or a cataract surgery and concurrent trabecular micro-bypass stent (iStent inject®) implantation (study group). The examination implied the analysis of IOP change, the number of glaucoma medication and visual acuity at the last follow-up visit. IOP was measured with Goldmann applanation tonometry. Visual acuity was described in LogMar scale. All data was analyzed by SPSS 22.0 and Mann-Whitney-U test was used to compare the two study groups.
Results:
The average follow-up time in the study group was 5.8±4.2 months but in the control group 3.1±2.2 months.
No statistically significant difference was seen between the study and control group comparing the patients’ age (67.5±9.1vs.67.4±8.8,p=1.0), IOP (median 18.0vs.16.0 mmHg,p=0.63), number of glaucoma medication (median 3 in both groups,p=0.86) and visual acuity (median logMAR 0.3vs.0.3,p=0.22) preoperatively.
At the last follow-up there was no statistically significant difference in IOP (15.5±3.2vs.16.1±3.5 mmHg,p=0.67) and visual acuity (median logMAR 0.1vs.0.2,p=0.11). Statistically significant lower number of glaucoma medication was seen in the study group (median 0 vs. 2,p=0.003) postoperatively.
Conclusions:
Cataract surgery combined with concurrent trabecular micro-bypass stent implantation not only improves visual acuity but also helps to obtain a better control over open-angle glaucoma.
Combined surgery resulted in statistically significant decrease in the number of glaucoma medication than cataract surgery alone, while IOP was not different in the two study groups.
