Toric IOL alignment: IntelliAxis-L (LensAR) vs Z Align (Callisto Eye)

Abstract Details

Purpose:

To compare femtosecond laser asisted capsular marking (IntelliAxis-L, LensAR, US) and digital marking (Z Align, Callisto Eye, Carl Zeiss Meditec, Germany) used for astigmatism axis marking in patients that underwent FLACS.

Setting:

Augentagesklinik Rheine, Rheine, Germany

Methods:

In patients with corneal astigmatism axis was marked on the anterior lens capsule with the help of IntelliAxis-L system and was compared with Z Align axis marking temple, which was imposed during surgery, while in non-astigmatic patients the axis was marked at 0o-180o with the same systems. Each time when using toric IOLs the alignment was done according to IntelliAxis-L markings. Intraoperative images were recorded and angle deviations between the two markings was measured using ImageJ bundled with Java 1.8.0_172.

Results:

Highly accurate and predictable toric IOL alignment was achieved with IntelliAxis-L compared to Z Align and a statistical difference between the two groups was encountered. In some cases, the Z Align function encountered deviations of more then 10o in axis marking, probably due to modified limbal vascular architecture caused by FLACS procedure.

Conclusions:

In our patients IntelliAxis-L showed precise capsular axis marking, slightly superior to Z Align, as the reference image is taken of the iris rather of the conjunctival and scleral vessels.
