Purpose:
To present a case of corneal melting 16 years after intracorneal ring segments (ICRSs) implantation and 9 years after corneal cross-linking (CXL) combined with photorefractive keratectomy (PRK) for keratoconus.

Results:
A male patient 57 y.o. presented with pain, redness and irritation in the right eye. The patient had undergone uneventful implantation of ICRSs for keratoconus 16 years ago and simultaneous PRK and CXL for visual acuity improvement and stabilization of corneal ectasia 9 years ago. According to our records he had undergone removal of the temporal ring segment because of corneal melting on the surface of the segment 8 months ago. Slit lamp examination revealed corneal melting on the surface of the remaining segment. The ring segment was removed along with treatment to promote corneal healing.
Conclusions:
Corneal melting remains a complication of ICRS, despite the presumed stabilization of the cornea and melting prevention using CXL treatment. Safety of combined PRK and CXL is yet to be studied for such cases.
