Abstract Details

Purpose:

To compare the Corneal Biomechanical Changes and the Endothelial Cell Density after Trabeculectomy and EX Press Valve implantation in 40 patients suffering from POAG and PEXG

Setting:

Prospective Interventional case series / Democritus University of Thrace, Greece

Methods:

Forty patients suffering from POAG or PEXG were in totally enrolled in this prospective study. Twenty patients were assigned in each group and underwent uneventful trabeculectomy and EX Press valve implantation. In all cases MMC was used. Corneal endothelial cells were measured with a Non-contact Specular Microscopy, Intraocular Pressure was measured by using both Goldmann applanation tonometry (GAT) and the Ocular Response Analyser (ORA), while IOP cc (corneal compensated IOP) and IOP gat (Goldmann correlated IOP) were evaluated. Central Corneal Thickness (CCT) and Axial Length (AL) were also measured.

Results:

With regard to postoperative changes, no differences in IOP, corneal endothelial cells and corneal biomechanics between groups were found. The IOP decreased by 8 and 8.4 mmHg, the corneal endothelial cells were decreased from 2465 + 255 to 2355 + 335 and from 2410 + 389 to 2315 + 355 in trabeculectomy and in EX PRESS group respectively. A significant change with regard to corneal biomechanical indices (for CH from 7.98 + 2.54 to 8.89 + 3.24 (p=0.005) and for CRF from 9.99 + 3.15 to 11.85 + 3.55 (p=0.005)) between preoperative and postoperative status in both groups were recorded.

Conclusions:

Both trabeculectomy and EX Press valve implantation appear to be safe and effective. Corneal biomechanical changes may influence the postoperative IOP measurement.
