Five-year follow-up of corneal thickness spatial profile, corneal volume distribution and total corneal refractive power changes after uneventful DMEK

Abstract Details

Purpose:

To investigate changes of corneal thickness spatial profile (CTSP), corneal volume (CV) distribution and corneal astigmatism (CA) using total corneal refractive power (TCRP) over a course of 5 years after uneventful Descemet membrane endothelial keratoplasty (DMEK)

Setting:

Department of Ophthalmology, Philipps University of Marburg, Germany

Methods:

Medical records of 212 consecutive DMEK surgeries were reviewed. Sixty cases without intraoperative and postoperative complications and with complete 5-year follow-up were included (group1). CTSP at corneal apex (CCT) and at 2mm, 4mm, 6mm and 8mm rings around the thinnest point (MCT), CV in 3mm, 5mm, 7mm and 10mm zones and TCRP in 2mm, 4mm 6mm and 8mm zones were evaluated preoperatively and at 3, 6, 12, 24 and 60 months postoperatively. The 60-month results were compared to an age-matched group of uncomplicated pseudophakic eyes (group2; n=20).

Results:

The CCT, MCT, and CTSP at 2, 4 and 6mm increased significantly at 5 years compared to 3 months outcomes (P<0.001). CV decreased significantly in all zones at 3 months (P<0.001), followed by a significant increase at 5 years in 3, 5 and 7mm zones (P<0.001). The CA showed in 2 and 4mm zones a continuous and significant decrease thought the examination period. The 5-year CCT, MCT and CTSP at 2mm were similar to group2 (P≥0.111), but CTSP at 4, 6, and 8mm were significantly higher in DMEK group (P≤0.023).

Conclusions:

Five years after uneventful DMEK, the corneal thickness within the central 2mm zone and CV in 3mm zone were similar to uncomplicated pseudophakic eyes. DMEK induced a continuous and significant decrease of astigmatism in the central 4mm zone throughout the 5-year postoperative course.
