Abstract Details

Purpose:

To report the clinical features and management of a case of cyclodialysis cleft following implantation of a glaucoma gel stent implant (XEN)

Setting:

University Hospital Ayr, Ayr, Scotland, UK

Methods:

A 62-year-old Caucasian male with a history of high myopia, bilateral pseudophakia and primary open angle glaucoma underwent a glaucoma gel stent implantation in the left eye for glaucoma control. The procedure was uncomplicated. Prior to his glaucoma surgical procedure his corrected visual acuity (CDVA) was 0.0 logMar in both eyes

Results:

Post operatively the left eye developed severe hypotony. Gonioscopy showed 3 clock hours of cyclodialysis cleft. The left eye vision was severely impaired to counting fingers due to hypotonic maculopathy. He underwent closed chamber surgical correction of his cyclodialysis cleft. Post operatively the hypotony resolved and the CDVA improved to 0.0 logMar

Conclusions:

Development of cyclodialysis is a rare unreported complication following glaucoma gel stent implant procedure. Closed chamber technique of closing the cyclodialysis cleft can be effective in reversing hypotony
