A prospective, randomized, eye-to-eye comparison of two wavefront-guided excimer laser systems for myopic LASIK: one year results

Abstract Details

Purpose:

To prospectively compare LASIK outcomes between two wavefront-guided excimer laser systems in the treatment of myopia. Outcome measures include high contrast snellen acuity, low contrast snellen acuity (25% and 5%), safety, efficacy, predictability and higher order aberration analysis at one year.

Setting:

University based academic refractive surgical service

Methods:

One hundred eyes of 50 consecutive patients were treated with wavefront-guided LASIK. One eye was treated with the Alcon Allegretto Wave Eye-Q 400 Hz excimer laser system and the fellow eye was treated with the AMO Visx CustomVue S4IR excimer laser system. Eyes were randomized according to ocular dominance. The mean pre-operative spherical equivalent refraction was -3.89 +/- 1.67 and -4.18 +/-1.73 in the Allegretto group and CustomVue group respectively. The mean cylinder was +0.83 +/-0.73 and +0.81 +/-0.77 in the Allegretto group and CustomVue group respectively. There were no statistically significant differences in pre-operative higher order aberrations between the two groups.

Results:

At post op month twelve, 100% of eyes in the Allegretto group and 92% of eyes in the CustomVue group were within 0.50 diopters of the intended correction. At the 12-month post-op visit the mean spherical equivalent refraction was -0.20 +/- 0.21 in the Allegretto group and -0.19 +/- 0.31 in the CustomVue group. At 12-months, the mean cylinder was reduced to +0.14 +/- 0.21 in the Allegretto group and +0.15 +/- 0.25 in the CustomVue group. At 12-months the mean total higher order RMS value in the Allegretto group decreased from 0.38 microns group to 0.33 microns. At 12-months the mean total higher order RMS value in the CustomVue group increased from 0.37 microns to 0.40 microns.

Conclusions:

Wavefront-guided LASIK using the Allegretto excimer laser system had better predictability and better patient self-reported clarity of vision at both day and night compared to the CustomVue excimer laser system. Eyes treated with the Allegretto excimer laser system experienced a reduction in total higher order aberrations compared to an increase in total higher order aberrations in eyes treated with the CustomVue excimer laser system. There were no differences between the two groups when comparing 5 and 25% low contrast visual acuity.
