Calculation of predictability of visual outcome following DSEK

Abstract Details

Purpose:

To assess the predictability of LogMar visual Acuity, graft thickness and total corneal thickness at different time intervals after Descemet Stripping Endothelial Keratoplasty.

Setting:

Southampton University Hospitals NHS Trust

Methods:

This prospective analysis included 78 consecutive cases of 74 patients that underwent manually dissected DSEK cases over a period of 44 months. Our initial 60 DSEK cases that were performed over a period of 24 months were excluded. Serial postoperative measurements of central graft, and total corneal, thicknesses were made using anterior segment optical coherence tomography. LogMar visual acuity was also recorded at each post-operative visit. The correlation between total corneal thickness and LogMar Visual Acuity was calculated

Results:

Graft thickness reduced sharply between the first post-operative day and 30 days post-op (p<0.05). After that the reduction in graft thickness continued for a year but with a much slower rate. Total corneal thickness and LogMar visual Acuity showed a near linear relationship, showing high predictability between all the time intervals apart from the first post-operative day. The power of the study was at 99.8%. Similarly graft thickness and LogMar visual acuity showed a nearly linear relationship with high predictability between all time intervals except the first post-operative day. The power of this analysis was 93.2%. All patients with logMar Visual Aquity greater than 0.355 belong to the postoperative period 90-365 days (or >3months or between 3 and 12 months).

Conclusions:

LogMar Visual acuity improves greatly during the first post-operative. Graft thickness and total corneal thickness show a near linear correlation with LogMar visual Acuity with a high predictability score. This allows us to predict the patientÂ’s final visual acuity even after the first post-operative month with a high level of precision.
