Prevalence of subclinical keratoconus in family members of patients with keratoconus

Purpose:

To determine the prevalence of subclinical keratoconus among the relatives of patients with keratoconus in a French population

Setting:

Service ophtalmologie, unité segment antérieur, centre national de référence du Kératocōne, bātiment T, place Amélie-Raba-Léon, 33000 Bordeaux, France

Methods:

In a prospective comparative study, 218 eyes of 109 relatives of 46 patients with keratoconus (KCN) were measured with a dual-scheimpflug analyzer and compared to a control group, including 210 normal eyes of 116 refractive surgery candidates. The prevalence of topographic subclinical keratoconus was then calculated in each group and analyzed based on the degree of relationship.

Results:

Subclinical KCN was diagnosed in 41.5% (n = 45) of the family members compared to 8.4% (n = 7) in the control group (p < 0.001). The prevalence of subclinical KCN among the family members of patients with KCN was 8.2 times greater than in the control group (95% CI [3,24; 20,61]). No difference in the distribution of subclinical KCN was found between subgroup of family members (p = 0.4), parents, siblings, or children. Subclinical KCN were found in parents of patient in 56% when only one of the two parents was evaluated compared to 80% when both parents were analyzed (p < 0.001).

Conclusions:

Family members of patients with keratoconus have a higher prevalence of subclinical keratoconus. Therefore, caution should be taken when considering keratorefractive surgery procedures in this population.
