Abstract Details

Purpose:

The Incise® intra-ocular lens (IOL) is made of a material customized for micro-incision cataract surgery (MICS). A prospective evaluation was conducted to evaluate wound stretch and lens delivery through a sub 2mm incision, with the injector cartridge tip inside the eye.

Methods:

The Viscoject® 1.5 cartridge has an outer diameter allowing it to be inserted through a 1.8mm incision. A prospective clinical study including 100 patients was conducted. In the 29 patients already enrolled coaxial micro-incision cataract surgery (C-MICS) using the Stellaris® phacoemulsification system was performed through an incision that measured from 1.7 to 1.9mm. The Incise® IOL was implanted with the Viscoject® 1.5 inserter tip inside the anterior chamber. Incision sizes were measured using gauges before and after IOL implantation. The behavior of the IOL during implantation was evaluated.

Results:

C-MICS was performed uneventfully in all cases. With no enlargement of the incision required, the Incise® IOL was implanted using the conventional injection technique. IOL powers ranged from 17.0 to 27.0D. Mean incision size was 1.90 ± 0.09mm (Range: 1.7 – 2.0 mm). Mean stretch was 0.0± 0.14 mm: no stretch in 17 of 29 cases, 0.1 mm in 9, 0.2 mm in 2cases and 0.3 mm in 1 (the first surgery) case. All IOL deliveries were successful with no IOL or cartridges damage. No complication related to the IOL or injector was noted. Learning curve using the new cartridge was short. The unfolding of the lens was well controlled, allowing easy placement in the capsular bag.

Conclusions:

Incise® IOL implantation with the Viscoject® 1.5 injector using the conventional injector-tip-inside-the-eye technique did not cause any significant stretch of the corneal wound. IOL performance during implantation and placement in the capsule was controlled and predictable. C-MICS with Incise® IOL implantation is a safe procedure.
