Evaluation of corneal biomechanical response as a function of intraocular pressure using dynamic infrared signal analysis and dynamic Scheimpflug imaging

Abstract Details

Purpose:

To evaluate corneal biomechanical structure using ocular response analyzer (ORA) and Corvis ST data.

Setting:

Shinagawa LASIK Center, Tokyo, Japan.

Methods:

1262 eyes of 795 patients enrolled in our study. 3 groups were established, according to the intraocular pressure, group I (range 10-13mmHg), group II (range 14-17mmHg), and group III (range 18-21mmHg). Each group included 3 subgroups, as a basis of which corneal thickness measurements was used, subgroup 1 (range 465-510?), 2 (range 510-555?), and 3 (range 555-600?). Corneal hysteresis (CH), corneal resistance factor (CRF), and corneal compensated intraocular pressure (IOPcc) were recorded with ORA. Additional corneal biomechanical parameters from Corvis ST were used. The central corneal thickness (CCT) was measured by using an ultrasound pachymeter.

Results:

CH?and CRF from ORA, applanation time1, 2, and deformation amplitude from Corvis ST showed significant differences between similar subgroups of each IOPcc groups (p < 0.0001). IOPcc is correlated negatively with CH (r?-0.38, p < 0.0001) and positively with CRF (r?-0.075, p?0.0076). There are a positive correlation of IOPcc with applanation time 1 (r?0.44, p < 0.0001), applanation velocity 2 (r ?0.195, p < 0.0001), radius (r = 0.111, p < 0.0001), and negative with applanation time 2 (r? - 0.54, p < 0.0001), applanation velocity 1 (r = - 0.118, p < 0.0001) and deformation amplitude (r? - 0.362, p < 0.0001).

Conclusions:

The analysis of ORA and Corvis ST parameters are indicated as very informative for the evaluation of corneal biomechanical structure. IOP parameters should be taken into consideration.
