Abstract Details

Purpose:

Evaluate ultrasound times using the Victus femtosecond laser using a novel technique of translenticular hydrodissection.

Setting:

Private ambulatory surgical centre

Methods:

This single surgeon study compared the first consecutive 108 cases of lensectomy (cataract and refractive lens exchange) surgery using the Victus femtosecond laser to perform capsulotomy and lens fragmentation to 107 consecutive conventional immediately prior to adoption of the technology. After the first 28 cases, a method of translenticular hydrodissection (TLH) was developed to assist lens mobilization and lens dismantling. Ultrasound times (average power (AP), ultrasound time (UST) and effective phacoemulsification time (EPT) were compared between the Victus group and conventional lens surgery. Further analysis of the Victus group compared the first 28 eyes where no hydrodissection was performed with those in whom TLH was performed. Data was statistically analyzed using paired t- tests.

Results:

This single surgeon study compared the group of 108 consecutive femtosecond laser lens surgery cases [with or without trans-lenticular hydrodissection (TLH) to assist lens mobilization and lens dismantling] to the group of 108 consecutive conventional cataract procedures performed immediately prior to adoption of the femtosecond laser technology. Average ultrasound power (AP), ultrasound time (UST) and effective phacoemulsification time (EPT) were compared between the femtosecond group and the conventional lens surgery group. Further analysis of the femtosecond group compared the 81 eyes with TLH to 27 eyes without TLH.

Conclusions:

: The AP, UST and EPT were significantly longer in the conventional lens surgery group compared to the femtosecond laser group: 7.30±2.56% vs. 5.32±2.48% (p=0.000), 9.89±5.32secs vs. 8.58±4.66secs (p=0.044), 0.93±1.00secs vs 0.57±0.51secs (p=0.002) respectively. In the femtosecond laser group, these parameters were significantly higher in eyes without TLH: 5.78±2.23% vs. 5.16±2.56% (p= 0.046), 10.95±4.66secs vs 7.78±4.41secs (p=0.046),and 0.72±0.26secs vs. 0.52±0.53secs (p=0.002), respectively.
