Abstract Details

Purpose:

To determine the effectiveness of the Catapulse System (MED-LOGICS INC) in combination with the cataract femtosecond laser to achieve independence of eyeglasses in ametropic and presbiopic patients over 40 years of age.

Setting:

Mendez Vision Institute, Tijuana, Baja California, Mexico

Methods:

30 consecutive patients with ametropia and presbyopia were selected. Routine preopereative exams that included endothelial cell count (TOPCON), interferometry (IOL MASTER ZIESS), anterior segment tomography (OCULUS PENTACAM) were performed in all patients. Cataract Femtosecond Laser (LENSX ALCON) was used. 4.9 mm Capsulotomy, 6 segment nucleus fracture and one 1.5 mm central cylinder were used. Two opposite 1.2 mm stab wound incisions with 32 degree angle and one 2.2 mm three segment main incision are made using the laser. Hidrodisection, hidrodelamination is performed. Using the bimanual controlled pulsed vacume of the catapulse system each of the nucleus sections are absorbed. Bimanual cortex aspiration and a multifocal single piece intraocular lens (RESTOR ALCON) is placed in the capsular bag. One month postoperative refraction and endothelial cell count was performed in all patients.

Results:

24 eyes are hyperopic average +2.50 D. 6 eyes are myopic average -3.50 D. Keratometric cylinder average 0.75 D. 22 eyes had multifocal intraocular lens 8 patients had multifocal toric intraocular lens All patients had clear corneas and minimal inflammation in the anterior chamber in their next day visit. No complications in 30 cases. One month postoperative endothelial cell loss was 5.7 %. Average post operative refraction at one month +0.50 D.

Conclusions:

The procedure is similar to phacoemulsification but without ultrasound energy, no metal tip, low flow controlled anterior chamber, exact blade-less incisions, reproducible 4.9 mm centered capsulotomy. Using the femtosecond laser in combination with the pulsed vacuum cataract removal system is an efficient, exact, reproducible and safe combination for refractive lensectomy.
