Abstract Details

Purpose:

To develop a toric marker which can be used easily for small deep set eyes with narrow lids.

Setting:

Mitsui Memorial Hospital, Tokyo

Methods:

A reference toric marker with an electronic level device was made to mark 0 and 180 degree positions. Tilting of the device is notified by the five LED lights and beeping sound. According to the angle of tilting, red or orange LED will light with frequent or less frequent beeping sound. When the device is held completely horizontal, the green LED will light and stop beeping. The sensitivity of the device can be adjusted by 5 steps with the maximum accuracy of 0.2 degrees. After opening the lids with a speculum, the final axis will be marked with an intraoperative marking device. Two types of the devices were designed to mark the final axis.

Results:

As the marking blades are long and bent properly, it can easily reach the limbus of deep set eye with narrow lids to make the reference marks quite easily. As the sound and light indicate the horizontal position of the device, the surgeon can concentrate on the patientÂ’s eye without any need of paying attention to the unstable air bubble in the glass tube. Contrary to the conventional ring shaped intraoperative axis gauge which was placed on the limbus or on the bulbar conjunctiva, the new axis gauges were designed much smaller to place on the cornea that the final axis marking was easily attained even in the narrow surgical field.

Conclusions:

Electronic toric markers attained precise axis marking quite easily and quickly. The combination of the electronic reference marker and intraoperative axis markers enabled accurate marking even in the complicated narrow lids and deep set eyes.
