Collagen cross-linking for keratoconus in young patients ( less than 18 years of age)

Abstract Details

Purpose:

To compare the outcome of collagen cross linking in patients who were less than 18 years of age to those who were more than 18 years of age.

Setting:

Maxivision Eye Hospital, Hyderabad

Methods:

We retrospectively reviewed all the files of patients who underwent CXL for Keratoconus between June 2011 to June 2012. The patients were divided into 2 groups for analysis. The group 1 included patients under 18 yrs of age and group 2 more than 18 years of age. All these patients had a standard procedure using Dresden protocol with regular Riboflavin. Best spectacle corrected visual acuity(BSCVA), manifest refraction, orbscan changes and clinical picture were recorded before surgery and at 6 months.

Results:

The group 1 included 14 eyes of 10 patients ( 5 males and 5 females) with an mean age of 14.1 years( range 10 – 17 yrs) and group 2 included 13 eyes of 12 patients ( 6 males and 6 females) with an average age of 20.4 years ( range 18 – 27 yrs).There was significant improvement of BSCVA from a preoperative mean of 0.25 ( 0.00 to 0.30) to 0.14 ( 0.00 to 0.30) post – operatively in group 1, whereas in group 2 there was no significant change in the BSCVA from a preoperative mean of 0.18 ( 0.00 to 0.30) to 0.14 ( 0.00 to 0.30). There was 1.5 D of reduction in K value in the patients under 18 yrs of age versus 1D reduction in K value in patients more than 18 years of age. All eyes developed minimal faint stromal haze that cleared within 1 month. Progression of disease was not seen in any of the treated eyes within 6 months of follow up.

Conclusions:

Riboflavin-UVA corneal cross-linking resulted in better post op BSCVA and more reduction of K value in patients younger than 18 years of age as compared to patients more than 18 years of age.
