Femtosecond laser with built-in OCT system for corneal surgery

Abstract Details

Purpose:

To report on the safety and efficacy of using a femtosecond laser with built-in Optical Coherence Tomography (OCT) for corneal surgery.

Setting:

Shinagawa LASIK Center, Tokyo, Japan. Department of Ophthalmology, Wenzhou Medical College, Wenzhou, China.

Methods:

Three hundred eyes underwent LASIK, 300 eyes underwent post-LASIK corneal inlay implantation, and 5 eyes underwent corneal ring implantation. The corneal image was visualized using the built-in OCT system after placing a suction ring on the cornea. The following were verified; Bowman’s layer for LASIK patients, the location of the previous LASIK flap relative to the new pocket for post-LASIK corneal inlay patients, and the endothelial cell layer for corneal ring patients. Additionally, the postoperative corneal image was visualized after creating the LASIK flap/corneal pocket.

Results:

For the LASIK patients, Bowman’s layer was visualized with the preoperative OCT image and a safe distance between Bowman’s layer and the flap interface was confirmed. For the patients who underwent post-LASIK corneal inlay implantation, the original flap was visualized with the preoperative OCT image and a safe distance between the flap and the pocket was confirmed. For the corneal ring patients, the endothelial cell layer was visualized with the preoperative OCT image and a safe distance between the endothelial cell layer and the ring pocket was confirmed. There were no complications and all flaps and pockets were created safely.

Conclusions:

The built-in OCT of this femtosecond laser enables surgeons to visualize and confirm safe surgical parameters prior to conducting LASIK, post-LASIK corneal inlay implantation and ring implantation surgeries.
