A 12 month retrospective analysis of visual and optical outcomes in femtosecond LASIK using Advanced Customvue Wavefront Guided Ablation for compound myopic astigmatism with high cylinder

Abstract Details

Purpose:

To measure and analyse the visual and optical outcomes in Femtosecond Lasik for compound myopic astigmatism with high cylinder using Advanced CustomVue wavefront guided ablation with iris registration.

Setting:

Cornea & Refractive Service, Shroff Eye Centre, A-9, Kailash Colony, New Delhi - 110048, India

Methods:

1031 eyes with primary compound myopic astigmatism and a cylindrical power of over 2.50 dioptres had uneventful LASIK with femtosecond flap creation with iFS 150 Khz laser and excimer ablation with the Advanced CustomVue VISX StarS4 IR excimer laser. Visual, refractive and aberrometric outcomes were evaluated at 12 months follow up. Vector analysis of refractive astigmatism was performed. The patients were followed for 12 months after surgery. The primary outcome measures compared were uncorrected visual acuity, residual refractive error, predictability, amount of higher order aberrations, contrast sensitivity and overall patient satisfaction. The higher order aberrations (HOA) were measured from the anterior corneal surface map of topography (Oculus Pentacam) for a 4 mm pupil diameter before and 12 months after surgery.

Results:

This study included 1031 eyes. The significant reduction in refractive sphere and cylinder at 6 months and 12 months post operatively (p=<.01) was associated with improved distant visual acuity (UDVA) (p=<.01). 93.2% eyes had a spherical equivalent within ± 0.50D. 4.3 % eyes were retreated. There was no significant induction of higher order aberrations (HOA). All eyes showed a reduction in pre-existing HOA RMS error with an overall mean reduction of 63.7 % (p<.001). All visual and optical outcomes remained stable over this period. The targeted and surgically induced astigmatism magnitudes were 4.17 D and 3.83 D respectively. The Correction Index was 0.918. Safety and Efficacy indices were high.

Conclusions:

At 12 months, Femtosecond LASIK for primary compound myopic astigmatism with high cylinder >2.50 D performed with Advanced Customvue wavefront guided ablation and Iris registration is safe, effective and predictable, did not cause significant changes in higher order aberrations (HOA), or contrast sensitivity, with high patient satisfaction.
