Computerized Scheimpflug densitometry as a measure of corneal optical density after keratoplasty

Abstract Details

Purpose:

To investigate prospectively the values of corneal density using the Pentacam Scheimpflug system (Oculus, Inc., Wetzlar, Germany) after keratoplasty.

Setting:

Ophthalmology university department

Methods:

A Pentacam system was used to study corneal densitometry in subjects after penetrating keratoplasty (PKP), deep anterior lamellar keratoplasty (DALK) or endothelial keratoplasty (DSAEK). Corneal densitometry readings were recorded at 7 days, 1 and 3 months during the postoperative course. Eyes of patients with no prior corneal surgery served as controls. The values for peak and average for anterior stromal density and posterior stromal density, at the level of the graft interface in case of lamellar surgery, were chosen for analysis.

Results:

Both anterior and posterior stromal densitometry values in the 3 keratoplasty groups were significantly greater than in the control group (p<0.01). 75 % of PKP showed a normal stromal average density and 25 % showed an anterior peak in corneal density. In 90 % of DALK and 60 % of DSAEK, an double density peak was observed. Corneal densitometry values obtained by the Oculus Pentacam were signifcantly higher in patients after keratoplasty in all groups. Corneal densitometry appears to correlate with corneal interface haze in patients undergoing lamellar keratoplasty.

Conclusions:

The Pentacam may be a useful tool to quantify interface haze in the postoperative course of lamellar keratoplasty. Additional study in a larger patient population is needed to further evaluate the utility of the Pentacam in grading interface haze.
