Abstract Details

Purpose:

To report the refractive and visual outcomes of phacoemulsification and toric intraocular lens (IOL) implantation in patients with cataract and high postkeratoplasty astigmatism.

Setting:

Retrospective review

Methods:

Phacoemulsification and AcrySof Toric IOL (Alcon Laboratories, Inc., Fort Worth, TX) implantation was performed in one eye in 4 consecutive patients. Age, pre- postoperative best corrected visual acuities (BCVA) and refractive cylinder power, IOL axis shift, corneal graft clarity were recorded.

Results:

The mean age of the patients was 35,2 (range 25-42) year. All patients underwent penetrating keratoplasty for keratoconus. Preoperative BCVAs were 20/100, 20/100, 20/40 and 20/80 and increased to 20/25, 20/25, 20/20 and 20/25 in patients 1, 2, 3, 4 respectively after the surgery. The mean preoperative and postoperative refractive astigmatism were 5,18±1,14 D (range 3,5-6D) and 1,81±0,23 D (1,5-2 D) respectively. Two of four eyes had SN60AT8 toric IOL model, while two had SN60AT9 IOL model. The corneal grafts were transparent at postoperative 6 months. In all eyes, the mean IOL rotation was less than 5 degrees at last visit.

Conclusions:

Patients with cataract and high postkeratoplasty astigmatism may be good candidates for toric IOL implantation, and toric IOLs provide rotational stability in these patients.
