Advantages of using a dual-system phacomachine

Abstract Details

Purpose:

To evaluate practical advantages in MICS technique by using a phacomachine with dual pump (Venturi and peristaltic pumps), dual linear foot pedal control and dual ultrasound motion tip (longitudinal and transversal) during a single procedure.

Setting:

S. Bassiano Hospital, Dept. of Ophthalmology, Bassano del Grappa, Italy

Methods:

A series of 20 eyes of 10 patients were randomly selected to undergo cataract surgery. Phacoemulsification was performed with Signature (Abbott) by using both peristaltic pump and Venturi pump switching back and forth between two pumps during the same procedure with dual linear foot pedal control. Elliptical ultrasounds were delivered by using a tip with simultaneous dual motion. Parameters for peristaltic were flow 44 cc/min, Vacuum 450 mmHg, US 25%, bottle height 95 cm. For Venturi were vacuum 300 mmHg, US 20%, bottle height 105 cm

Results:

With peristaltic pump the dual linear foot pedal managed to control ultrasound delivery and flow simultaneously and gently while dual linear foot pedal delivered and controlled ultrasound at any level of vacuum with Venturi pump. The efficiency of this single integrated system was indicated by achieving a better followability with low levels of ultrasound energy even due to the elliptical motion. The average EFX value was 47.24±18 SD, the total average US power was 6.8%±1.85 SD and the total US time 61.4 sec.±28 SD.

Conclusions:

This dual-system machine allows surgeons to deal with any kind of cataract by using a single programme which offers a lot of combination during a single procedure. The followability is improved by combining the advantages of the two pumps and the two kinds of US stroke into one dual linear foot pedal control system. Using this combination optimal efficiency and safety can be achieved at any surgical time.
