Cataract surgery with femtosecond laser after Raindrop Near Vision inlay

Abstract Details

Purpose:

To report surgical outcomes in a patient that developed cataract 3 years after Raindrop Near Vision Inlay implantation. Cataract surgery was performed using a femtosecond laser.

Setting:

Slade & Baker Vision Center, Houston, TX, USA.

Methods:

Case Report. Patient implanted with the Raindrop Near Vision Inlay in April 2010, as part of the Raindrop US IDE Clinical Trial, and subsequently developed a posterior subcapsular cataract three years after Inlay implantation. The patient complained about decreased vision, glare and halo, and the slit lamp exam revealed a posterior subcapsular cataract located in the visual axis.

Results:

The patient underwent cataract surgery using a femtosecond laser (lenSx, Alcon) for capsulotomy (4.8 mm) and lens fragmentation (central cylinder with 3 vertical chop pattern) using the soft-fit patient interface. The Raindrop Near Vision Inlay did not interfere with Laser energy delivery. The capsulotomy was complete, without tags or areas uncut. Lens fragmentation chops were complete. During surgery, the corneal inlay allowed perfect visualization of the intraocular structures throughout the surgical procedure. A monofocal IOL targeted for distance acuity was implanted. Day 1 after surgery the patientÂ’s uncorrected visual acuity for distance was 20/20 and for near J2. Postoperative refraction matched preoperative target.

Conclusions:

The Raindrop Near Vision Inlay is a hydrogel (80% water content). It blends naturally with the cornea, maintaining its transparency. The inlay allowed effective and complete delivery of femtosecond laser energy into the capsule and nucleus, and did not interfere with visualization of intraocular structures, allowing for a safe cataract removal. IOL calculations after near vision inlay were predictable.
