Purpose:

Doing cataract surgery there is always the intention to reach the best refractive result. Patients more and more expect to reduce spectacle dependence. Aiming for spectacle independence certain kinds of intraocular lenses (IOL) have been designed. Hereby the so called multifocal IOL plays a significant role. To be precise most of these multifocal lenses are in fact bifocal IOL, because merely two focal points are generated. This leads to the fact, that most patients have a satisfactory distance and near visual acuity, but show a reduced intermediate vision. This was the impetus for the company Carl Zeiss Meditec to design a new trifocal IOL (AT LISA tri 839MP) to offer the patients an IOL with a good distance, near and intermediate vision. The aim of this prospective study is to evaluate the postoperative results of the trifocal intraocular lens AT LISA tri 839 MP for distance, intermediate and near visual acuity after bilateral implantation.

Methods:

This European multicentric study comprised a prospective series of 50 patients who underwent bilateral cataract surgery doing phacoemulsification and trifocal intraocular lens implantation in 100 eyes. Patients older than 50 years were included if they presented with a bilateral cataract, had a corneal astigmatism of lower than 1.0 diopter and had no other ocular disease like corneal scarring, Fuchs’ endothelial dystrophy, glaucoma, macular degeneration, diabetic retinopathy etc. Biometric measurements and IOL power calculations were performed with the IOLMaster (Carl Zeiss Meditec). Monocular and binocular far, intermediate and near visual acuities (logMAR) were measured preoperatively, 1 month and 3 months after surgery at 6 m, 80 cm and 40 cm respectively. Statistical analysis was performed with SAS Version 9.2 (Copyright© 2002-2008 by SAS Institute Inc., Cary, NC, USA.).

Results:

Mean monocular uncorrected visual acuity (VA) (± standard deviation [SD]) was 0.10±0.15, 0.06±0.30, and 0.20±0.17 for 6 m, 80 cm, and 40 cm respectively. Mean monocular best distance corrected VA (±SD) was 0.08±0.21, 0.05±0.25, 0.10±0.12 for 6 m, 80 cm, and 40 cm respectively. Mean binocular uncorrected VA (±SD) was 0.04±0.12, 0.00±0.23 and 0.15±0.14 for 6 m, 80 cm and 40 cm respectively. Binocular best distance corrected VA was 0.04±0.19, 0.00±0.26 and 0.06±0.11 for the mentioned distances. 98% of the patients reached an uncorrected binocular intermediate visual acuity of better or equal than 0.30 logMAR.

Conclusions:

Doing cataract surgery with phacoemulsification and implanting the AT LISA tri 839 MP is a safe and reliable procedure. The AT LISA tri 839MP intraocular lens provides satisfactory vision at intermediate distance without limiting near and far visual acuity. This new lens therefore broadens our range in the refractive tools to reach our goal to create spectacle independence.
