Abstract Details

Purpose:

To evaluate Long-terms results of AT LISA (Carl Zeiss Medite? ) implantation. To analyze vision acuity at near and long distance, to compare AC depth, AC angles width, IOP changes preop and postop. Find out the criteria of patient satisfaction after surgery.

Setting:

International Center for Ophthalmology , Moscow, Russia Centro Oculistico Capitanio, Milan, Italy

Methods:

study includes 422 yes of 211 patients with cataract , presbyopia, hyperopia, myopia. 6 patients with mild glaucoma Examination before the surgery: BUCVA,BCVA, IOP measurement , axial length , K readings and IOL power calculation (SRKT formula) by IOL-Master (Carl Zeiss, Meditec), Anterior segment OCT byVisante(Carl Zeiss, Meditec). Visual fields (Humphrey). All patients undergone standard phacoemulsification, under topic anesthesia with multifocal IOL (AT LISA Carl Zeiss Medite?) implantation Eyes were analyzed 1,3,6,12,24,36 month postoperatively.

Results:

The mean BUCVA improved from 20/50 preoperatively to 20/25 for far distance. At the first day after operation visual acuity was increased in 208 patient .One patient had 20/100 due to severe stromal edema of cornea; his eye was completely recovered during the 1st month with vision acuity 20/20. All the patients gained 20/30 average UCVA for near distance , and no changes were seen during all period of study. In 1 case glasses for near vision were needed, 5 patients need to use glasses for intermediate distance. 1patient , who had constant headache before the surgery reports of it disappearance with lens exchange. IOP reduced was noticed in 102 patients, one patient after three month postoperatively have IOP rise to 23-25mmHg (seemingly associated with thyroiditis ) 5 patients with glaucoma continued drop therapy. According Visante OCT AC depth increased and AC angles became wider in all the cases. All patients showed high satisfaction of their vision for far and near distance.

Conclusions:

Lens exchange to multifocal IOL AT LISA (Carl Zeiss Medite?) shows good predictability, gives high patientÂ’s satisfaction in near and distance vision acuity; additional glasses for medium distance don"t decrease patientÂ’s anticipation. AT LISA implantation is strongly recommended to the patients with cataract, presbyopia, narrow AC angles, shallow AC and increased IOP, as effective method for near and far vision restoration, IOP reduce and improve of fluidic flow because of AC anatomy changes. Can be used to prevent pupillary block in the eyes with close angle glaucoma.
