Ultrasound biomicroscopic analysis following Cionni ring and in-the-bag IOL implantation for subluxated lenses

Abstract Details

Purpose:

To assess the position of the Cionni ring, the Intra Ocular Lens(IOL) relative to the ring, eyelet of the ring and trans-scleral suture, in pediatric eyes undergoing subluxated lens surgery using Ultrasound Biomicroscopy (UBM).

Setting:

Iladevi Cataract & IOL Research Centre, Raghudeep Eye Clinic, Ahmedabad - 380052, India

Methods:

30 eyes of 17 children below 18 years of age, with Cionni ring and in-the-bag IOL implantation, and atleast six months postoperative follow-up were recalled for UBM analysis. All the evaluations were performed in the long axis of the IOL, with both haptics in view. Structures examined included the cornea, anterior chamber, iris, IOL position, Cionni ring position and position of the trans-scleral suture. Measurements included anterior chamber depth(ACD), anterior chamber angle width(ACA), optic-edge-iris distance, Cionni ring-iris distance, Cionni element-iris edge distance and Cionni element distance from the nearest ocular wall.

Results:

On Slitlamp, IOL was well-centered in all eyes. In two eyes IOL was partly in sulcus and partly in-the-bag. Cionni element was above anterior capsule in all eyes except one, where it was in-the-bag. UBM Findings: Mean ACD: 4.10±0.74 mm, mean ACA: 34.06˚±23.2˚. In eyes with part sulcus IOL placement, IOL was pushing iris forward leading to very narrow angles. Optic edge-iris distance was 0.48±0.23mm, Cionni ring-iris distance 0.61±0.32 mm on either side of pupil, Cionni element–iris distance 0.23±0.15mm and Cionni element - nearest ocular wall 2.85±0.57mm. Trans-scleral suture passed through the pars plicata in all eyes except one.

Conclusions:

UBM analysis of eyes with Cionni ring implantation provides valuable information on the exact location of the ring with respect to ocular landmarks. It revealed mild tilt of the IOL in all eyes, with adequate distance between posterior surface of iris and cionni ring and trans-scleral suture in most eyes.
