Intraocular light dispersion in pseudophakic patients with different IOL

Abstract Details

Purpose:

To evaluate the intra-ocular light scattering in pseudophakic patients implanted with three different intra-ocular lenses (IOL). A secondary goal of this study was to analyze the tear film scattering index in the three groups of patients.

Setting:

Clinica Oftalmológica Salgado-Borges - Porto - Portugal and CEORLab - Center of Physics - University of Minho – Braga - Portugal

Methods:

Seventy-one cataractous patients (age 69±8 years, range: 53 to 86 years; 60% females) undergone facoemulsification followed by pseudophakic IOL implantation with BL ENVISTA (n=18), AMO Tecnics ZCB00 (n=21 eyes) and AMO Tecnis ZA9003 (n=32 eyes) by the same experienced surgeon. Examinations were performed between 5 to 8 months after surgery. Despite being younger in the AMO ZCB00, patients did not presented significant differences regarding age, IOL power implanted or post-surgical refraction. Overall ocular scattering index for a 4mm pupil (overall OSI) and tear film ocular scattering index for a 7 mm aperture (tear film OSI) as well as other parameters relate to the quality of vision through the ocular-IOL optical system were evaluated with the Optical Quality Analizer System OQAS (Visiometrics, Spain) under non-dilated conditions.

Results:

: Average power of the lenses was 21.40±2.34D with a post-operative spherical equivalent refractive error of 0.14±0.51D of sphere and -0.18±0.51D of cylinder. Overall OSI was not significantly different between groups (ranging from 2.32±2.20 for AMO ZA9003 to 1.50±0.87 for AMO ZCB00, Mann-Whitney U test p>0.05). There was however a significantly lower profile width of the PSF at 50% of the total high with the AMOZCB00 which was significantly lower when compared with the AMO ZA9003 (4.28±1.53 vs 6.03±3.37, M-W U test, p<0.05). MTF cut-off point was not significantly different between lenses. Tear Film OSI indices were significantly different between lenses with patients in the AMO ZCB00 presenting the lower values compared to AMO ZCB00 (1.31±0.61 vs 2.40±2.14, M-W U test, p<0.05). ZCB00 group was also the one presenting the lower degradation in the inter-blink period with the two remaining groups showing a significant degradation in the inter-blink period (M-W U test, p≤0.05). This difference might be related with the younger age of this cohort.

Conclusions:

Tear film quality might impact the post-cataract quality of vision. The differences found between the three groups regarding the overall OSI index might be explained by the better tear quality (lower tear film OSI) found in the younger cohort. OQAS might be of help in identifying tear film problems that might increase the Overall OSI index in post surgical patients undergoing cataract extraction with IOL implantation.
