Torsional versus torsional + axial ultrasound (US) in aspiration of cataracts of different density

Abstract Details

Purpose:

To compare efficacy and safety of torsional ultrasound with IP and combined torsional with longitudinal ultrasound in aspiration of different nuclear density cataracts.

Setting:

Saint-Petersburg branch of State Institution Eye Microsurgery Complex named after S.N.Fyodorov

Methods:

75 patients underwent conventional microcoaxial (2.2mm) phacoemulsification using “stop & chop” technique with flared tip (bevel - 45 degree). After grooving one hemisphere of the lens was aspirated using torsional US with IP and another using torsional and axial US in combination (custom pulse program). Cumulative dissipated energy (CDE), aspiration time and estimated fluid used were evaluated after each hemisphere removal.

Results:

Soft nucleus were aspirated with the same speed at both type of US (p>.05). However, more energy (CDE) was dissipated after combination US was used (p<.05), presumably due to longitudinal US energy contribution. In hard nucleus all parameters were much higher when torsional US with IP was used. (p<.05). Insufficiency of torsional energy leads to increase aspiration time and consequently increase dissipated energy.

Conclusions:

Using flared tip in relatively soft nucleus we suggest torsional US with IP. In hard nucleus we prefer combination US.
