Abstract Details

Purpose:

The range of ultrasound power modulations implemented in the Stellaris system include multiple pulsing modes, fixed and waveform burst modes. The mechanical effects of these power modulations are poorly characterised and their use in removing dense hard cataracts is based on subjective surgeon preferences rather than a clear understanding of the performance of these modulations. These modulations are critical to safely remove dense cataracts. The objective of the current study is to Characterise the performance of various power modulations available on the Stellaris System in an in vitro model and optimise their use in removing dense cataracts.

Methods:

An in vitro model was developed to test the performance of pulse, fixed burst and waveform burst modes. The model used a very hard block of cheese as well as an almond to simulate two types of dense hard cataract. The performance of these three modulations was characterised for consistency and efficiency for sculpting, embedding, chopping and segment removal manoeuvres. The primary outcome measure was the amount of ultrasound energy used.

Results:

Pulse, fixed burst and waveform burst power modulations each produced a unique performance profile. These profiles provided particular advantages for sculpting, embedding, chopping and segment removal of the two invitro simulations used for a dense hard cataract i.e. a hard cheese and and almond. The waveform burst modulation provided a unique performance profile suited for embedding and chopping.

Conclusions:

Characterising the power modulation profiles available on the Stellaris system using an in-vitro simulation is a very useful tool in optimising their use in the removal of dense hard cataracts safely with the least ultrasound energy use.
