Purpose:
To present the different ways we use the femtosecond laser technology for creating pockets of different dimensions and shapes for a variety of intracorneal surgical procedures, such as intrastromal insertion of biocompatible inlays for the treatment of presbyopia, intracorneal direct drug delivery in cases of deep stromal keratitis and intracorneal silicon oil insertion in order to treat corneal oedema caused by bullous keratopathy.

Methods:
The intracorneal lens FlexivueTM Micro-Lens (Presbia, CA, The Netherlands) was implanted surgically inside a corneal pocket created in the non-dominant eye of 45 emmetropic, presbyopic patients using femtosecond laser. Intracorneal antibiotic therapy was delivered through a pocket of 1 mm width for the treatment of deep stromal keratitis, resistant to other therapeutic approaches. Insertion of transparent biocompatible hydrophobic material into the cornea was performed in the affected eye of five patients with bullous keratopathy and no light perception.
Results:
All surgical procedures were successfully accomplished without any intraoperative or postoperative complications. All patients remained free of symptoms during the one year follow up period.
Conclusions:
The use of the femtosecond laser technology is a safe and effective method in creating corneal pocket for different intracorneal procedures. The development of special software for customized pockets could further simplify and increase the efficacy of the procedure.
