Abstract Details

Purpose:

To compare and evaluate the Toric Implantable Collamer Lens (ICL) and Femtosecond LASIK for the correction of High Myopic Compound Astigmatism, and to assess post-operative visual outcomes, safety, predictability, and patient satisfaction in the two groups.

Setting:

Cornea & Refractive Service, Shroff Eye Centre, A-9 Kailash Colony, New Delhi - 110048, India.

Methods:

205 eyes underwent implantation of the STAAR Toric ICL and 231 eyes underwent Wavefront guided Femtosecond LASIK with the iFS 150 Khz laser and the Advanced CustomVue VISX StarS4 IR excimer laser for the correction of high myopic compound astigmatism. The manifest spherical equivalent (SE) was ? -8.0 D; the manifest refractive cylinder was ? 1.5 D. The safety, efficacy, predictability, stability and adverse effects were assessed post-operatively at 6 and 12 months.

Results:

At 12 months the mean safety index in the ICL group was 1.51 ± 0.17 and in the LASIK group 1.02 ± 0.35. The mean efficacy index was 0.91 ± 0.23 in the ICL group and 0.76 ± 0.11 in the LASIK group (p=<.01). 98.6% eyes in the ICL group and 84.3 eyes in the LASIK group were within ± 0.5 D of targeted spherical equivalent at 12 months (p = 0.031). There were no significant complications in the ICL group. 3 eyes in the LASIK group required enhancement ablations. The UDVA was better in the ICL group. The percentage of eyes gaining lines of UDVA was significantly higher in the ICL group (46.8%) versus the LASIK group (10.1%), (p=<.01). Contrast sensitivity (Vector Vision, CSV 1000E) at higher spatial frequencies was better in the ICL group. Patient satisfaction questionnaires showed high satisfaction in both groups.

Conclusions:

The STAAR Toric ICL performed better than the wavefront guided femtosecond LASIK in eyes with high myopic compound astigmatism in measures of safety, efficacy, predictability and stability, seeming an attractive alternative to corneal excimer laser surgery in these eyes.
